What is an anal fistula?
An anal fistula is an abnormal small channel that forms between the inside of the anal canal and the skin, passing through the anal sphincters around the anus. It most often follows a perianal abscess that has drained, leaving a persistent tract.
It manifests as discharge, seepage, sometimes pain, or recurrent abscesses. The fistula does not heal spontaneously and requires surgical management.
Why is fistula treatment delicate?
The fistula canal passes, in part, through the internal and external sphincter muscles that ensure continence. The entire challenge of treatment is to eliminate the fistula without damaging this muscle, in order to preserve continence.
This is precisely the advantage of minimally invasive laser techniques, which target the fistula tract while sparing neighboring structures. Traditional open surgery carries a significant risk of long-term postoperative incontinence.
FILAC Laser Treatment
The laser technique involves introducing a thin fiber into the fistula tract. This is ideally done after placement of a silicone seton, which keeps “the door ajar” to allow drainage of the abscess: it is therefore a two-stage procedure.
The delivered laser energy closes the canal from within, while preserving the sphincter. Performed on an outpatient basis, this approach limits painful aftereffects and the risk of continence impairment, compared to older techniques.
Fistula and Abscess: A Close Connection
Abscess and fistula are often two stages of the same condition: the abscess is the acute and infectious phase, the fistula the chronic sequel.
Proper management of the abscess reduces the risk of fistula, but does not always eliminate it. Also discover our management of anal abscesses.
