Breast Lipofilling Objectives

Breast lipofilling, also known as lipomodeling or breast lipostructure, is a surgical procedure that involves transferring fat cells from areas with excess (abdomen, inner thighs, hips, knees, back…) to the breast area.

Generally speaking, the lipofilling technique allows for significant volume modification without risk of inflammatory or allergic reaction, with the certainty that the natural product will always be well accepted by the body.

Furthermore, the advantage of breast lipofilling is benefiting from liposuction of the lower body, for a more harmonious silhouette.

Regarding breast lipofilling, the procedure allows for moderate breast augmentation – approximately half a cup size – and can only be considered if the patient’s request is to moderately enhance her décolletage.

Breast lipofilling can also be considered in combination with breast augmentation using implants (this procedure is called hybrid or composite breast augmentation), with the fat concealing the contours of the implants for slender patients who do not have sufficient tissue in the breast area, thus offering a more natural result even if the implant size is significant.

Who is breast lipofilling for?

Breast lipofilling is intended for patients with moderate breast hypotrophy, whether the cause is genetic, or following pregnancy, breastfeeding, weight loss, or in cases of moderate breast asymmetry.

The procedure can be considered from the age of 18.

Naturally, the patient must also have excess fat in the abdomen, hips, or thighs. It should be noted that the ratio between aspirated fat and usable fat for lipofilling is 1 to 3 (900 cc of aspirated fat is necessary to inject 300 cc of purified fat). With resorption averaging 30%, approximately 200 cc of fat will remain from the 900 cc initially harvested.

This indicates a breast augmentation of approximately half a cup size.

Before the intervention

The first consultation is essential to understand your request and your motivations. Depending on your morphology and your anatomy, your surgeon will determine his operating strategy.

The goal is to achieve a result suited to your silhouette and meeting your expectations. During this consultation, it will be important to carefully assess the quality of the skin and glandular tissue.

Your surgeon will perform a clinical examination to ensure the absence of any abnormality. A radiological and preoperative health assessment will be conducted according to prescriptions.

Anesthesia

Breast lipofilling is generally performed on an outpatient basis under general anesthesia or local anesthesia with deep sedation, as several anatomical sites are involved during the same operative session.

A consultation with your anesthesiologist will take place a few days before your procedure.

The day of the procedure

Your surgeon will adapt his operating strategy according to your particular case in order to obtain the best possible result.

During the first phase of the procedure, the surgeon will harvest fat using a non-traumatic micro-cannula from the harvesting areas defined during consultation with the patient at the sites of excess fat.

The choice of harvesting sites depends on the quantity of fat deemed necessary and the available harvesting sites.

The fat is centrifuged to collect only the fat cells (adipocytes).

The surgeon then makes micro-incisions of 1 to 2 mm in the inframammary fold, then reinjects the fat using fine cannulas in several layers, into the mammary gland up to beneath the skin, then between the gland and the pectoralis major muscle, into the pectoralis major muscle, and finally between the muscle and the ribs.

The duration of the procedure depends on the number of donor sites and the amount of fat to be transferred. It may vary from 1 hour 30 minutes to 3 hours depending on the case.

After the procedure

Pain is generally moderate, more significant in the aspirated areas, and will be relieved by simple analgesics.

Any swelling in the harvesting areas and breasts disappears between 1 and 3 months, while bruising that most often appears in the harvesting areas disappears after a few weeks.

A compression garment will be prescribed. It must be worn for 6 weeks over the harvesting areas. In some cases, a compression bra may also be prescribed. Some fatigue may be experienced for one to two weeks, especially in cases of significant fat harvesting.

Refraining from sports is recommended for 6 weeks, resumption of socio-professional activities is possible after one week.

 

Results

The result can be appreciated from the first month and the final result will be visible after 6 months, once the fat that has not taken is resorbed.

This procedure generally brings great satisfaction to patients who benefit from a significant increase in their breasts, with improved breast contour.

Since the grafted fat is living and dependent on weight variations, the patient must ensure not to lose weight to maintain her breast volume. The breasts will tend to increase if, on the contrary, the patient gains weight.
